S-5637, S-5638 & S-5639 on blue 'herald' labels, which feature the 1974-style tall narrow headers and all bold non-italic font. Black and white cover with white WRC logo on front of box, white cardboard lining. Issued again in November 1976, November 1977, May 1978 and May 1979. Issued with 16-page libretto - unknown if this is common to all copies, but here, the sheet with pages 3-4-14-15 has not been stapled or attached, and sits loose and out of place (placed in correct order here).
